People with multiple sclerosis who for one year followed a plant-based diet very low in saturated fat had much less MS-related fatigue at the end of that year — and significantly less fatigue than a control group of people with MS who didn’t follow the diet, according to an Oregon Health & Science University study being presented today at the American Academy of Neurology’s annual meeting in Philadelphia, Pa.
The study was the first randomized-controlled trial to examine the potential benefits of the low fat diet on the management of MS. The study found no significant differences between the two groups in brain lesions detected on MRI brain scans or on other measures of MS. But while the number of trial participants was relatively small, study leaders believe the significantly improved fatigue symptoms merited further and larger studies of the diet.
"Fatigue can be a debilitating problem for many people living with relapsing-remitting MS," said Vijayshree Yadav, M.D., an associate professor of neurology in the OHSU School of Medicine and clinical medical director of the OHSU Multiple Sclerosis Center. "So this study’s results — showing some notable improvement in fatigue for people who follow this diet — are a hopeful hint of something that could help many people with MS."
The study investigated the effects of following a diet called the McDougall Diet, devised by John McDougall, M.D. The diet is partly based on an MS-fighting diet developed in the 1940s and 1950s by the late Roy Swank, M.D., a former head of the division of neurology at OHSU. The McDougall diet, very low in saturated fat, focuses on eating starches, fruits and vegetables and does not include meat, fish or dairy products.
The study, which began in 2008, looked at the diet’s effect on the most common form of MS, called relapsing-remitting MS. About 85 percent of people with MS have relapsing-remitting MS, characterized by clearly defined attacks of worsening neurological function followed by recovery periods when symptoms improve partially or completely.
The study measured indicators of MS among a group of people who followed the McDougall Diet for 12 months and a control group that did not. The study measured a range of MS indicators and symptoms, including brain lesions on MRI brain scans of study participants, relapse rate, disabilities caused by the disease, body weight and cholesterol levels.
It found no difference between the diet group and the control group in the number of MS-caused brain lesions detected on the MRI scans. It also found no difference between the two groups in relapse rate or level of disability caused by the disease. People who followed the diet did lose significantly more weight than the control group and had significantly lower cholesterol levels. People who followed the diet also had higher scores on a questionnaire that measured their quality of life and overall mood.
The study’s sample size was relatively small. Fifty-three people completed the study, with 27 in the control group and 22 people in the diet group who complied with the diet’s restrictions.
"This study showed the low-fat diet might offer some promising help with the fatigue that often comes with MS," said Dennis Bourdette, M.D., F.A.A.N., chair of OHSU’s Department of Neurology, director of OHSU’s MS Center and a study co-author. "But further study is needed, hopefully with a larger trial where we can more closely look at how the diet might help fatigue and possibly affect other symptoms of MS."

Blue light may fight fatigue around the clock
Researchers from Brigham and Women’s Hospital (BWH) have found that exposure to short wavelength, or blue light, during the biological day directly and immediately improves alertness and performance. These findings are published in the February issue of Sleep.
"Our previous research has shown that blue light is able to improve alertness during the night, but our new data demonstrates that these effects also extend to daytime light exposure," said Shadab Rahman, PhD, a researcher in BWH’s Division of Sleep Medicine and lead author of this study. "These findings demonstrate that prolonged blue light exposure during the day has an an alerting effect."
In order to determine which wavelengths of light were most effective in warding off fatigue, the BWH researchers teamed with George Brainard, PhD, a professor of neurology at Thomas Jefferson University, who developed the specialized light equipment used in the study. Researcherscompared the effects of blue light with exposure to an equal amount of green light on alertness and performance in 16 study participants for 6.5 hours over a day. Participants then rated how sleepy they felt, had their reaction times measured and wore electrodes to assess changes in brain activity patterns during the light exposure.
The researchers found that participants exposed to blue light consistently rated themselves as less sleepy, had quicker reaction times and fewer lapses of attention during the performance tests compared to those who were exposed to green light. They also showed changes in brain activity patterns that indicated a more alert state.
"These results contribute to our understanding of how light impacts the brain and open up a new range of possibilities for using light to improve human alertness, productivity and safety," explained Steven Lockley, PhD, neuroscientist at BWH and senior investigator of the study. "While helping to improve alertness in night workers has obvious safety benefits, day shift workers may also benefit from better quality lighting that would not only help them see better but also make them more alert."
Researchers note that the next big challenge is to figure out how to deliver better lighting. While natural light is ideal, many people do not have access to daylight in their schools, homes or work places. In addition to improvements in daylight access, the advent of new, more controllable lighting technologies may help enable researchers to develop ‘smart’ lighting systems designed to maximize the beneficial effects of light for human health, productivity and safety.
Red Light Increases Alertness During “Post-Lunch Dip”
Acute or chronic sleep deprivation resulting in increased feelings of fatigue is one of the leading causes of workplace incidents and related injuries. More incidents and performance failures, such as automobile accidents, occur in the mid-afternoon hours known as the “post-lunch dip.” The post-lunch dip typically occurs from 2-4 p.m., or about 16-18 hours after an individual’s bedtime from the previous night.
A new study from the Lighting Research Center (LRC) at Rensselaer Polytechnic Institute shows that exposure to certain wavelengths and levels of light has the potential to increase alertness during the post-lunch dip. The research was a collaboration between Mariana Figueiro, LRC Light and Health Program director and associate professor at Rensselaer, and LRC doctoral student Levent Sahin. Results of the study titled “Alerting effects of short-wavelength (blue) and long-wavelength (red) lights in the afternoon,” were recently published in Physiology & Behavior journal.
The collaboration between Figueiro and Sahin lays the groundwork for the possible use of tailored light exposures as a non-pharmacological intervention to increase alertness during the daytime. Figueiro has previously conducted studies that show that light has the potential to increase alertness at night. Exposure to more than 2500 lux of white light at night increases performance, elevates core body temperature, and increases heart rate.
In most studies to date, the alerting effects of light have been linked to its ability to suppress melatonin. However, results from another study led by Figueiro demonstrate that acute melatonin suppression is not needed for light to affect alertness during the nighttime. They showed that both short-wavelength (blue) and long-wavelength (red) lights increased measures of alertness but only short-wavelength light suppressed melatonin. Melatonin levels are typically lower during the daytime, and higher at night.
Figueiro and Sahin hypothesized that if light can impact alertness via pathways other than melatonin suppression, then certain wavelengths and levels of light might also increase alertness during the middle of the afternoon, close to the post-lunch dip hours.
During the study conducted at the LRC, participants experienced two experimental lighting conditions in addition to darkness. Long-wavelength “red” light (λmax = 630 nanometers) and short-wavelength “blue” light (λmax = 470 nanometers) were delivered to the corneas of each participant by arrays of light emitting diodes (LEDs) placed in 60 × 60 × 60 cm light boxes. Participant alertness was measured by electroencephalogram (EEG) and subjective sleepiness (KSS scale).
The team found that, compared to remaining in darkness, exposure to red light in the middle of the afternoon significantly reduces power in the alpha, alpha theta, and theta ranges. Because high power in these frequency ranges has been associated with sleepiness, these results suggest that red light positively affects measures of alertness not only at night, but also during the day. Red light also seemed to be a more potent stimulus for modulating brain activities associated with daytime alertness than blue light, although they did not find any significant differences in measures of alertness after exposure to red and blue lights. This suggests that blue light, especially higher levels of blue light, could still increase alertness in the afternoon. It appears that melatonin suppression is not needed for light to have an impact on objective measures of alertness.
“Our study suggests that photoreceptors other than the intrinsically photosensitive retinal ganglion cells respond to light for the arousal system,” said Figueiro. “Future research should look into the spectral sensitivity of alertness and how it changes over the course of 24 hours.”
Sahin, who has more than 10 years of experience in railway engineering, was interested in this study from a transportation safety perspective, and what the results could mean to the transportation industry. “Safety is a prerequisite and one of the most important quality indicators in the transportation industry,” said Sahin. “Our recent findings provided the scientifically valid underpinnings in approaching fatigue related safety problems in 24 hour transportation operations.”
From the present results, it is not possible to determine the underlying mechanisms contributing to light-induced changes in alertness because the optical radiation incident on the retina has multiple effects on brain activity through parallel neural pathways. According to Figueiro, that is an area that she would like to explore in future research.
Why your brain tires when exercising
A marathon runner approaches the finishing line, but suddenly the sweaty athlete collapses to the ground. Everyone probably assumes that this is because he has expended all energy in his muscles. What few people know is that it might also be a braking mechanism in the brain which swings into effect and makes us too tired to continue. What may be occurring is what is referred to as ‘central fatigue’.
"Our discovery is helping to shed light on the paradox which has long been the subject of discussion by researchers. We have always known that the neurotransmitter serotonin is released when you exercise, and indeed, it helps us to keep going. However, the answer to what role the substance plays in relation to the fact that we also feel so exhausted we have to stop has been eluding us for years. We can now see it is actually a surplus of serotonin that triggers a braking mechanism in the brain. In other words, serotonin functions as an accelerator but also as a brake when the strain becomes excessive," says Associate Professor Jean-François Perrier from the Department of Neuroscience and Pharmacology, who has spearheaded the new research.
Help in the battle against doping
Jean-François Perrier hopes that mapping the mechanism that prompts central fatigue will be useful in several ways. Central fatigue is a phenomenon which has been known for about 80 years; it is a sort of tiredness which, instead of affecting the muscles, hits the brain and nervous system. By conducting scientific experiments, it is possible to observe and measure that the brain sends insufficient signals to the muscles to keep going, which in turn means that we are unable to keep performing. This makes the mechanism behind central fatigue an interesting area in the battle against doping, and it is for this reason that Anti Doping Danmark has also helped fund the group’s research.
"In combating the use of doping, it is crucial to identify which methods athletes can use to prevent central fatigue and thereby continue to perform beyond what is naturally possible. And the best way of doing so is to understand the underlying mechanism," says Jean-François Perrier.
Developing better drugs
The brain communicates with our muscles using so-called motoneurons. In several diseases, motoneurons are hyperactive. This is true, for example, of people suffering from spasticity and cerebral palsy, who are unable to control their movements. Jean-François Perrier therefore hopes that, in the long term, this new knowledge can also be used to help develop drugs against these symptoms and to find out more about the effects of antidepressants.
"This new discovery brings us a step closer to finding ways of controlling serotonin. In other words, whether it will have an activating effect or trigger central fatigue. It is all about selectively activating the receptors which serotonin attaches to," explains Jean-François Perrier.
"For selective serotonin re-uptake inhibitor (SSRI) drugs which are used as antidepressants, we can possibly help explain why those who take the drugs often feel more tired and also become slightly clumsier than other people. What we now know can help us develop better drugs," concludes Jean-François Perrier.
